New Patient Registration
The surgery are once again accepting a limited number of new patient registration requests (TN23 and TN24 postcodes ONLY within our boundary).
Due to the limited number of new patient registrations we can accept, all new patients should complete the relevant ADULT and/or CHILD Request for registration forms below and submit via email to: firstname.lastname@example.org, where upon approval, patients will receive New Patient Registration Forms via the post. These forms should be completed, signed and returned to the surgery within 48 hours.
Please allow 14 days to receive your Registration form/s, before contacting the Surgery for information.
If we exceed our weekly quota of requests for new registrations, those who are unsuccessful, will be prioritised for the next week.
We hope that you will be happy with the services that we provide, however you do have the right to change your doctor by approaching the surgery of your choice and requesting acceptance onto their patient register. Your medical records will then be forwarded via the local primary care agency.
Equally if patients are threatening, violent or abusive towards any member of staff within the surgery they will be removed from the patient register.
To Select/Check a box on the form, double click the box with your mouse button and select Checked. At this stage of registration, we will accept a digital signature on the form.
